Introduction Financial conflicts of interest (fCOI) present well documented risks to the integrity of biomedical research. However, few studies differentiate among fCOI types in their analyses, and those that do tend to use preexisting taxonomies for fCOI identification. Research on fCOI would benefit from an empirically-derived taxonomy of self-reported fCOI and data on fCOI type and payor prevalence.Methods We conducted a content analysis of 6,165 individual self-reported relationships from COI statements distributed across 378 articles indexed with PubMed. Two coders used (...) an iterative coding process to identify and classify individual fCOI types and payors. Inter-rater reliability was κ = 0.935 for fCOI type and κ = 0.884 for payor identification.Results Our analysis identified 21 fCOI types, 9 of which occurred at prevalences greater than 1%. These included research funding (24.8%), speaking fees (20.8%), consulting fees (18.8%), advisory relationships (11%), industry employment (7.6%), unspecified fees (4.8%), travel fees (3.2%), stock holdings (3.1%), and patent ownership (1%). Reported fCOI were held with 1,077 unique payors, 22 of which were present in more than 1% of financial relationships. The ten most common payors included Pfizer (4%), Novartis (3.9%), MSD (3.8%), Bristol Myers Squibb (3.2%), AstraZeneca (3.1%), GSK (3%), Boehringer Ingelheim (2.9%), Roche (2.8%), Eli LIlly (2.5%), and AbbVie (2.4%).Conclusions These results provide novel multi-domain prevalence data on self-reported fCOI and payors in biomedical research. As such, they have the potential to catalyze future research that can assess the differential effects of various types of fCOI. Specifically, the data suggest that comparative analyses of the effects of different fCOI types are needed and that special attention should be paid to the diversity of payor types for research relationships. This paper explores why respondents to a telephone public-opinion survey often give reasons for answering as they do, even though reason-giving is neither required nor encouraged and it is difficult to see the reasons as attempts to deal with disagreement. We find that respondents give reasons for the policy claims they make in their answers three times as frequently as they give reasons for value or factual claims, that their reasons tend to involve appeals to personal experience, and that they (...) often talk about their thought processes, especially when the evidentiary stakes are high. We then explore several ways of explaining these findings. We suggest that one useful approach is to see the reason-giving in the survey interviews as deliberative, reflexive argumentation of the sort described as `critical thinking. We further suggest that the reason such argumentation is often conducted out loud in the interviews, rather than internally, is that it functions in the service of rhetorical ethos, in particular the need to display the fact that one is human, with human autonomy and agency. Doing this may be particularly important in contexts such as anonymous survey interviews in which people are at risk of being treated like machines. (shrink)

Current treatments for chronic pain have limited benefit. We describe a resilience intervention for individuals with chronic pain which is based on a model of viewing chronic pain as dysregulated homeostasis and which seeks to restore homeostatic self-regulation using strategies exemplified by survivors of extreme environments. The intervention is expected to have broad effects on well-being and positive emotional health, to improve cognitive functions, and to reduce pain symptoms thus helping to transform the suffering of pain into self-growth. A total (...) of 88 Veterans completed the pre-assessment and were randomly assigned to either the treatment intervention or control. Fifty-eight Veterans completed pre- and post-testing. The intervention covered resilience strengths organized into four modules: engagement, social relatedness, transformation of pain and building a good life. A broad set of standardized, well validated measures were used to assess three domains of functioning: health and well-being, symptoms, and cognitive functions. Two-way Analysis of Variance was used to detect group and time differences. Broadly, results indicated significant intervention and time effects across multiple domains: Pain decreased in present severity [F = 5.02, p < 0.05, η2p = 0.08], total pain over six domains [F = 14.52, p < 0.01, η2p = 0.21], and pain interference [F = 6.82, p < 0.05, η2p = 0.11]; Affect improved in pain-related negative affect [F = 7.44, p < 0.01, η2p = 0.12], fear [F = 7.70, p < 0.01, η2p = 0.12], and distress [F = 10.87, p < 0.01, η2p = 0.16]; Well-being increased in pain mobility [F = 5.45, p < 0.05, η2p = 0.09], vitality [F = 4.54, p < 0.05, η2p = 0.07], and emotional well-being [F = 5.53, p < 0.05, η2p = 0.09] Mental health symptoms and the cognitive functioning domain did not reveal significant effects. This resilience intervention based on homeostatic self-regulation and survival strategies of survivors of extreme external environments may provide additional sociopsychobiological tools for treating individuals with chronic pain that may extend beyond treating pain symptoms to improving emotional well-being and self-growth.Clinical Trial Registration: Registered with ClinicalTrials.gov. Ackelsberg investigates women’s activist participation in the National Congress of Neighborhood Women, a Brooklyn association established in 1974–75, which she treats as a model of democratic civic engagement that incorporated differences while avoiding the exclusions of the past. The NCNW assisted poor and working class women in organizing to better meet their needs and those of their communities. It arose in response to the ways women were either ignored or belittled when they attempted to engage in political work both in (...) their communities and beyond. In working with each other, the women found that they needed to address issues of diversity. The programs they created to help build bridges across differences that helped facilitate their successful activism while, simultaneously, broadening their understanding of what constitutes “politics.”. Theories of social justice are necessarily abstract, reaching beyond the particular and the immediate to the general and the timeless. Yet such theories, addressing the world and its problems, must respond to the real and changing dilemmas of the day. A brilliant work of practical philosophy, Frontiers of Justice is dedicated to this proposition. Taking up three urgent problems of social justice neglected by current theories and thus harder to tackle in practical terms and everyday life, Martha Nussbaum seeks (...) a theory of social justice that can guide us to a richer, more responsive approach to social cooperation. The idea of the social contract--especially as developed in the work of John Rawls--is one of the most powerful approaches to social justice in the Western tradition. But as Nussbaum demonstrates, even Rawls's theory, suggesting a contract for mutual advantage among approximate equals, cannot address questions of social justice posed by unequal parties. A pathbreaking new study of women and morality How do people decide what is "good" and what is "bad"? How does a society set moral guidelines -- and what happens when the behavior of various groups differs from these guidelines? Martha Saxton tackles these and other fascinating issues in Being Good , her history of the moral values prescribed for women in early America. Saxton begins by examining seventeenth-century Boston, then moves on to eighteenth-century Virginia and nineteenth-century St. Louis. (...) Studying women throughout the life cycle -- girls, young unmarried women, young wives and mothers, older widows -- through their diaries and personal papers, she also studies the variations due to different ethnicities and backgrounds. In all three cases, she is able to show how the values of one group conflicted with or developed in opposition to those of another. A history of American women's moral life thus gives us a history of women's emotional life as well. The aim of this article is to show how a suspect’s statement travels through two stages of the criminal law process: the police interrogation and the trial, exhibited by two modes of production: talk and writing. I first discuss how the suspect’s statement is elicited and written down by the police in the police report; next I consider how the police report is made to form part of a legally adequate case-file; and finally I investigate the ways in which the (...) judge quotes and refers to the police report in his questioning of the suspect during the trial. This step-by-step inspection of the trajectory of the suspect’s statement shows processes of de- and recontextualization. The suspect’s statement is written down so as to enable it to be taken out of one context and inserted into another. This means that old meanings are removed from the suspect’s statement and new meanings are added. In the courtroom, however, the judge treats the suspect’s written statement as his own individual production, irrespective of the interactional environment in which it was elicited. We propose that the fundamental mechanism underlying resilience is the integration of novel or negative experiences into internal schemata. This process requires a switch from reactive to predictive control modes, from the brain's salience network to the default mode network. Reappraisal, among other mechanisms, is suggested to facilitate this process.

The year 2024 is proving to be a landmark in the explosion of diseases all over the world. Collectively, we are experiencing a huge rise in obesity and related diseases like diabetes type 2 and heart disease; a huge rise in mental illnesses, leading to deaths and diseases of despair; a huge increase in the gap between the world’s billionaires and everyone else; a huge increase in authoritarian populist leaders replacing democratic political candidates. The Olympic Games are part of the (...) national identity of almost every nation in the world. They were institutionalized as a way to encourage each citizen to cultivate a healthy mind in a healthy body. This paper begins with the view of a healthy civilization underlying all aspects of the Olympic Games. The underlying view also follows Aristotle’s model of a flourishing citizen in a flourishing society. A number of recent books that focus on some aspect of the many diseases people experience today, including physical and mental health, refer explicitly to Aristotle’s model of flourishing as the standard against which they expose corruptions and a model to follow as we create a healthier culture in the future. The World Congress in Rome, the Center for Values in Philosophy and the ISUD also support the work of scholars who bring together ideas about how to create a healthier international civilization so that future generations can survive and thrive. More than a century after Guido Adler's appointment to the first chair in musicology at the University of Vienna, Music, Criticism, and the Challenge of History provides a first look at the discipline in this earliest period, and at the ideological dilemmas and methodological anxieties that characterized it upon its institutionalization. Author Kevin Karnes contends that some of the most vital questions surrounding musicology's disciplinary identities today-the relationship between musicology and criticism, the role of the subject in analysis and (...) the narration of history, and the responsibilities of the scholar to the listening public-originate in these conflicted and largely forgotten beginnings. Karnes lays bare the nature of music study in the late nineteenth century through insightful readings of long-overlooked contributions by three of musicology's foremost pioneers-Adler, Eduard Hanslick, and Heinrich Schenker. Shaped as much by the skeptical pronouncements of the likes of Nietzsche and Wagner as it was by progressivist ideologies of scientific positivism, the new discipline comprised an array of oft-contested and intensely personal visions of music study, its value, and its future. Karnes introduces readers to a Hanslick who rejected the call of positivist scholarship and dedicated himself to penning an avowedly subjective history of Viennese musical life. He argues that Schenker's analytical experiments had roots in a Wagner-inspired search for a critical alternative to Adler's style-obsessed scholarship. And he illuminates Adler's determined response to Nietzsche's warnings about the vitality of artistic and cultural life in an increasingly scientific age. Through sophisticated and meticulous presentation, Music, Criticism, and the Challenge of History demonstrates that the new discipline of musicology was inextricably tied in with the cultural discourse of its time. Nussbaum defends literacy and education for women as a crucial condition for lessening many of the problems that women face worldwide, such as abusive marriages, inadequate jobs, and poor health, which restrict women’s capacities to engage in citizenship practices. Nussbaum’s proposal extends to secondary and higher education and particularly urges the development of women’s critical faculties and imagination. At present, the commitments of poorer nations and states, as well as those of wealthy nations, their citizens, and their corporations are woefully (...) inadequate to serve women’s needs. Female education is sometimes opposed on the grounds that it destroys non-literate cultures, which have their own values; yet such cultures may harbor misery and injustice and their norms may even be opposed by the women in the cultures. In lieu of an abstract, here is a brief excerpt of the content:MIMETIC VIOLENCE AND NELLA LARSEN'S PASSING: TOWARD A CRITICAL CONSCIOUSNESS OF RACISM Martha Reineke University ofNorthern Iowa In her recent essay, "Working through Racism: Confronting the Strangely Familiar," Patricia Elliot proposes that members of dominant groups who want to contest racism1 not only challenge economic, political, and social processes within society that produce racism, but also address personal claims they make on institutional structures which help to maintain (...) it (63). Sympathetic with an anti-racist strategy that concerns itself not only with racism at the institutional level but also with racism on a more intimate scale, Elliot nevertheless laments that individual complicity in racism is frequently attributed to aproblematic attitude. As a consequence, anti-racist work among members of dominant groups often is arrayed along a narrow spectrum: in workshops or in-service seminars participants attempt to identify and acknowledge biased and discriminatory views. Elliot fears that consciousness-raising efforts ofthis kind are unlikely to effect social change ifthose who participate continue to favor the confessional stance. Urging her readers to develop a "critical consciousness," a concept she takes from bell hooks (hooks 118), Elliot asks that members ofdominant groups who desire to promote social change not only combat institutional racism, but also divest themselves oisubjective investments in racism (63). Only as persons commit 1 Racism "involves the subordination ofpeople ofcolor by white people" (Rothenberg 6). Marked by discrimination and prejudice, racism entails also the exercise ofpower (conscious and unconscious, intentional and unintentional) by which white people maintain positions of privilege over persons ofcolor (Rothenberg 7). Martha Reineke75 themselves to a sophisticated and nuanced analysis of white complicity in structures of domination in order to grapple with racism at multiple levels—includingidentityformation, maintenance, and transformation—can members ofdominant groups actually tackle and begin to dismantle it (Elliot 63). Elliot's call to critical consciousness is laudable, but the goal she sets forth is not easily attained. As 1 struggle to achieve hook's critical consciousness of racism, like Elliot (64), I find difficult the task ofexploring the construction ofmy own identity as a white academic.2 For example, when I discuss issues in women's lives in the classroom or at professional conferences, I not infrequently am challenged by women of color who note that unexamined privilege (race and class) skews my perspective.3 1 also find myselfspeaking for others, assuming their voices, when I should speak only for myself. As a consequence, I regularly have cause to ponder whether, under the guise of contributing to anti-racistwork, I am appropriating others' voices in ways that reinforce racism rather than challenge it. For instance, on what grounds do I, a white woman, in this essay propose to address the problem of racism by reflecting on a novella written by an African-American woman? Does white privilege foreground my choice, precluding me from reading a work of literature in ways that might challenge rather than further racism? Grappling with these issues while reflecting on the theme of a critical consciousness ofracism, I find instructive the Girardian notion of mimesis. The dynamics of mimesis account well for agents of racism who, simultaneously recognizing and denying difference, act in ways that inhibit social and subjective transformation. This paper describes many connections between the wisdom literature of the Ancient Greeks and the work of contemporary scholars, intellectuals and professionals in many fields. Whether or not they use the word nous to refer to the highest power of the human soul, I show that their views converge on the existence of such a power. The paper begins with a brief summary of Greek educational texts, including Greek mythology, Homer, tragedy, and Plato’s dialogues, showing that they are designed to (...) educate the power of mind. Usually without realizing it, many later schools of thought can be shown to come to conclusions that are consistent with the insights of one school of thought or cultural practice among the Ancient Greeks. Many other ancient cultures also had a holistic view of the cosmos, the human soul, and the best human life. (shrink)

This paper summarizes Ervin Laszlo’s worldview in The Systems View of the World: A Holistic Vision for Our Time.1 Laszlo claims that current discoveries in the sciences have led to a different model of the physical world, human nature, and human culture. Instead of the models formulated during the Enlightenment, according Systems thinkers “systems interact with systems and collaboratively form suprasystems”. This view has led to a reexamination of: 1) each academic discipline; 2) the relationship between disciplines; 3) the nature (...) of theory and its relation to practice; 4) the relationship between religion and the sciences; 5) of the nature of the social sciences and our ability to develop a universal, normative ethic; 6) the relation between reasoning, emotion and imagination. The evolution of the reflective self-consciousness unique to homo sapiens has led to the formation of cultures. Cultures must be understood assuprasystems that emerged from natural systems and are dependent upon them. Given this universal natural foundation, systems thinkers are recognizing the common patterns between nature and culture and between different cultures. The examination of systems has also shown us that the suprasystems of culture create a level of complexity and reality over and above the natural world and can even destroy themselves and their own natural foundationFrom the perspective of the ISUD, this view means it is possible, natural, and necessary for academics to engage in meaningful dialogue with each other, showing how the ways they have been trained to examine “reality,” or “truth,” can be integrated. Further, professional academics should be able to talk to non-academics, to people in leadership roles, and to all human actors. Since it is a fact that individuals are parts of many larger wholes, the ISUD can nurture the process of the development of reflective self-consciousness in the formation of an international culture, an emerging suprasystem.Laszlo calls this sphere of spiritual interaction, with its physical foundation, a noosphere, his word for a “meeting of the minds.” Given our collective destruction of natural systems, it is imperative that human beings develop some version of a Systems view of reality. ISUD should work to foster this development, even though the professional training of individuals will call the process by other names, based on the labels of the past. (shrink)
